Financial Aid Forms

Forms listed below are grouped by academic year. Please choose your form from the correct academic year.

 Looking for your Tax Form 1098-T?

Most forms can be completed electronically using the 鈥淪ubmit Webform鈥� link next to the document name shown below. If you do not see a webform link provided, please contact the One-Stop Center at financialaidoffice@lowercolumbia.edu, Meet us online in Zoom (M-F 8am-5pm), or call  (360) 442-2322 to request the form be sent to you for signature. If you prefer to complete the document by hand, you can click on the Webform link, choose "Options" once the form is open, then select "Download PDF" to print the form and submit a paper copy to the One-Stop Center for processing..

Most Financial Aid forms are Academic year specific. Please be sure to review the dates provided below and choose the appropriate timeframe before submitting a document.

  The 2026-27 Academic year is July 1, 2026 - June 30, 2027. 

  The 2025-26 Academic year is July 1, 2025 - June 30, 2026. 

Did you know there are multiple ways to submit your financial aid forms?

  • During office hours, you can visit us in person at the One Stop Center in the Admissions building.
  • Submit the form via the drop box located on the south side of the Admission building. Look for the "Students Services/Financial Aid Drop Box鈥� posted sign.
  • Submit the form via fax to (360) 442-2379.
  • Mail the form to: 黑料社, Attn: Financial Aid Office, PO Box 3010, Longview, WA 98632
